Assessment and planning

The Assessment and Planning Process was the foundational activity and guide for organizations to identify the needs of and existing activities related to promoting and improving the prevention and early detection of breast, cervical and colorectal cancer among underserved women and men served by and living within the service area of their organization.
All organizations funded for the Cancer Prevention and Early Detection Program before 2019 (Request for Applications numbers 6342 and 8374) completed an Assessment and Planning Tool in the first year of their contract. Organizations should use the results of the Assessment and Planning Tool to select which strategies (Targeted Community Outreach, Clinic Quality Improvement, Health Navigation and Clinical Services) to apply for in their annual continuing application and budget.
As of June 30, 2019 updates to assessment and planning work have been incorporated into the annual continuing application process (See Section 1: Grant Management of the Program Manual for additional information about continuing application timelines).
In future Requests for Applications, the Assessment and Planning Tool will be incorporated into the application process.

For reference, copies of the questions that were included on the Tool are found below.
  • Individual health clinic (includes provider-owned clinic).
  • Federally Qualified Health Center.
  • Community health center.
  • Hospital.
  • Hospital networks for the use and benefit of a hospital within their network (may include hospital foundations).
  • Safety net clinic.
  • Rural health center.
  • Local public health agency.
  • A support, network or member organization responding to the RFA on behalf of one or more health systems providing preventative services.
  • None of the above.
