Health navigation and clinical services

The Health Navigation and Clinical Services (HNCS) strategy is also known as the Women's Wellness Connection or WWC.

Strategies

There are two sub-strategies within the Health Navigation and Clinical Services strategy:

  • Health Navigation and Clinical Services sub-strategy for eligible uninsured/underinsured clients (required).
    • Organizations are required to implement (or plan for implementation of) the Health Navigation and Clinical Services sub-strategy for uninsured/underinsured clients.
  • Health Navigation sub-strategy for eligible insured clients (optional).
    • The Health Navigation sub-strategy for insured clients is optional and not all organizations will be funded to implement (or plan for implementation of) this sub-strategy.

Health Navigation

  • Funds navigation of eligible clients into timely and complete breast and cervical cancer screening and diagnostic services.
  • Reduces disparities by helping eligible clients overcome barriers.
  • It can be provided to eligible insured, uninsured, or underinsured clients. 

Clinical Services

  • Funds breast and cervical cancer screening and diagnostic procedures for eligible uninsured or underinsured clients.
  • Reduces disparities by reducing out-of-pocket costs of screening for eligible uninsured or underinsured clients.
  • Cannot be implemented without also implementing Health Navigation for this same group of clients.
