A lawful presence affidavit is required for enrollment in Health First Colorado's Breast and Cervical Cancer Treatment Program (BCCP). A completed lawful presence affidavit must be included with the initial BCCP documents faxed to CDPHE for review before granting approval to call and get a presumptive eligibility (PE) number.
Organizations must receive the following from clients applying for BCCP:
- A valid form of identification.
- An affidavit stating the recipient is lawfully present.
