Client requirements
Organizations must receive the following two documents from clients applying for enrollment in Health First Colorado's Breast and Cervical Cancer Treatment Program (BCCP):
- A valid form of identification.
- An affidavit stating the recipient is lawfully present.
- Lawful presence is satisfactory proof that the applicant's presence in the United States is authorized under federal law. A completed lawful presence affidavit must be included with the initial BCCP documents faxed to CDPHE for review before approval to call and get a presumptive eligibility (PE) number.
