Progress reports

Progress reports provide CDPHE an understanding of progress toward contract deliverables and may identify implementation challenges or technical assistance needs, allowing organizations to adjust and continuously improve their work. All organizations must submit online progress reports for all funded strategies twice per year according to the following schedule:

Progress reports will be announced in the CPED Weekly Update when available. Primary program contacts will also receive a notification email with a unique URL that should be used to complete the report. Organizations are responsible for completing progress reports on time as a contract requirement. Organizations will access progress reports using the unique link sent to the primary program contact(s) listed in the Agency Management System (AMS). Links are customized to specific strategies and/or programs funded so all individuals who receive a link should complete the progress report survey. Organizations must respond to all CDPHE inquiries related to progress reports.
